p2PSA、phi 与前列腺癌早期筛查和诊断

前列腺穿刺活检是诊断前列腺癌的金标准，但是作为侵入性检查方式，穿刺活检存在感染和出血等风险。前列腺癌筛查的无创手段包括：直肠指检、超声、CT、MRI，以及血清 PSA 等。但是，由于 PSA 具有前列腺组织特异性，却没有肿瘤特异性，在某些良性前列腺疾病中亦有增高，其应用受到一定的限制。尤其是在 PSA 处于灰区（4-10ng/mL）的人群中，前列腺癌穿刺阳性率仅为 26%2，却有 0.1-7% 的机会受到感染 3。

为了帮助实现前列腺癌精准医疗，贝克曼库尔特公司上市了 p2PSA 试剂盒，并推出了前列腺健康指数，phi（prostate health index， phi =（p2PSA / fPSA ) *√PSA）。在中国即将注册上市。

在 Clinical Chemistry 上的一项研究显示：所有 PCa 患者组的 p2PSA 比 fPSA 百分率（%p2PSA）以及 phi，比非 PCa 患者组有非常显著的升高。相对于 %p2PSA，p2PSA，%fPSA 或 PSA，phi 的 ROC 曲线下面积最大，对 PCa 的预测有着更好的临床表现，提高了预测准确性 4。

更重要的是，phi 可以帮助区分侵袭性和非侵袭性前列腺癌：更高的 phi 意味着罹患侵袭性前列腺癌的风险也更高。在 Gleason 分值 7 的患者里，可以观测到 phi 的中位值比 Gleason 分值<7 的患者有着显著的升高。而且恶性前列腺癌患者（Gleason 分值 7）phi 值也相对较高（表 1）4。另一项来自 Guazzoni 的研究还显示，phi 可以帮助临床医生在手术前预测前列腺癌病人的临床分期、肿瘤体积和 Gleason 分值，以帮助评估手术方案 5。

为了减少患者痛苦、节约医疗资源，血清 p2PSA、phi 还可以减少过度穿刺和避免不必要的穿刺，一项研究显示：当 phi 的切点值设定在 24 时，41% 的前列腺穿刺可以避免 6。

对于肿瘤体积较小，未扩散和 Gleason 分值较低的隐匿性前列腺癌，往往并不行前列腺切除术，而只是保持监测。对于有较大风险发展为侵袭性前列腺癌的病人，需要重复穿刺以积极监测肿瘤的发展。但是，怎样挑选出这部分病人是一个难题。有文献指出：隐匿性前列腺癌病人的 phi 的基准值和变化值，都可以用于预测肿瘤的发展 7,8。另外，在已经接受了前列腺切除术的病人中，p2PSA 也可以帮助区分生化复发（PSA 升高但是肿瘤并没有复发）和真正复发的病人 9。

2016 年 NCCN 前列腺癌早期检测指南（第二版），推荐「PCA3、phi、4K Score」，为前列腺癌早期检测的推荐指标 10。phi 的应用最为广泛，已经得到美国 FDA 的批准。
